Medical Treatment Consent

In the event reasonable attempts to contact me have been unsuccessful, I hereby give my consent for (1) the administration of any treatment deemed necessary by above -named doctor, or, in the event the designated preferred practitioner is not available, by another licensed physician or dentist; and (2) the transfer of the child to any hospital reasonably accessible.

This authorization does not cover major surgery unless the medical opinions of two other licensed physician or dentists, concurring in the necessity for such surgery, are obtained prior to the performance of such surgery. Facts concerning the child's medical history including allergies, medications being taken, and any physicals impairments to which a physician should be alerted.

Field Trip/ Activity Permission Statement

I/We the parent(s)/ guardian(s) of ( Your Student), give permission for my/our child to attend Emmanuel Christian Academy's Summer Enrichment STEAM Camp, to participate in all field trips and activities on and off campus that are provided. I/We understand the nature of the field trips and activities that are planned and agree to the procedures and policies governing the field trips and activities.

I/We understand that Emmanuel Christian Academy contract with independent providers and organizations to provide services for the for the Summer Enrichment STEAM Camp and extend this agreement to all activities supervised by all employees/ staff/ volunteers during those periods of time.
